Course Description

Today, more than 14 million Americans are alive with a history of cancer; all grappling with a complicated mix of post-treatment issues. In 2006, the Institute of Medicine recommended that every cancer patient receive an individualized survivorship care plan to help improve the quality of care survivors receive as they live with, through and beyond cancer. In this course participants will learn how to access, create and benefit from a cancer survivorship care plan.

Course Objectives

Upon completion of this course, learners will be able to:

  • Define cancer survivorship and important post-treatment concerns of survivors
  • Name the decisions and events that led to new national standards for survivorship care plans
  • Describe benefits and key components of an effective survivorship care plan
  • Outline practical ways for health care providers, family caregivers and cancer survivors to optimize the benefits of survivorship care plans
  • List resources that can help with creating a cancer survivorship care plan

    Jane Meier Hamilton MSN, RN

    Jane Meier Hamilton, RN, MSN, is an entrepreneur, consultant, educator and author. A nurse for 40 years and family caregiver for 20, she is the Founder of Partners on the Path, LLC,(www.PartnersonthePath.com), a certified Woman Owned Business which provides caregiver support programs to businesses and non-profits, health-care and faith-based organizations.

    Jane is a content expert on managing caregiver stress, building resilience and preventing compassion fatigue. She has created original, research-based resources that help professional and family caregivers remain healthy, effective and compassionate. Her book, The Caregiver’s Guide to Self-Care is available in print, 1-hour audio, and e-book formats.

    Jane holds a BSN from Marquette University; her MSN in Psychiatric and Community Mental Health Nursing is from the University of Illinois. She has consulted with more than 150 organizations and presented over 3,000 workshops and keynote addresses nationwide. Jane is a member of Sigma Theta Tau and has been listed in Who’s Who in American Nursing.
